I'm not sure how fighting when charged from flank and rear should work. Lets consider two scenarios 1. A gets charged by B.
Which units from A are in base contact in 1 or 2? Should they shuffle left to right in 1 or front to back in 2?
When units from A die where they should be taken from? Since normally you replace fighting troops in the front by taking them from the back, should you take them from left in 1 and from front in 2? Do they stay turned side/back to the enemy until reformed and enemy gets +1/+2 in all subsequent round of combat?

Question 1: you shift the last rank of A into contact with unit B. Models of unit C that therefore would not be in btb with can still attack across the gap.
BRB p. 49 (Incomplete Ranks): "If a unit is fighting to its flank, the models in the incomplete rank are moved into contact with the enemy, there to fight and be fought normally. If the unit is fighting to its rear (or fighting to both flanks) some enemy models might not end up in base contact because of the models in the incomplete rear rank. In this specific situation, the models can fight across the gap, even if not physically in base-to-base contact. In reality, the chargers would not have stopped one step away from the enemy and would d have moved in to continue the fight — treat these models as being in base contact with the enemy."
Question 2: you still take casualties from A's rear rank, and, if need be, you nudge the enemy unit (or A) back into btb.
BRB p. 61 (Shrinking units and multiple fights): "Occasionally, a situation can arise when one or more units are no longer in base contact with the enemy, but at least part of the enemy unit is still alive. This normally occurs when an enemy has been charged in the rear — as casualties come from the rear rank first, this would in theory leave the attackers stranded from their foe. Clearly this is wrong, just as warriors on a real battlefield would push on to continue the fight, and would not stop fighting if the enemy was standing a few paces away, so must warriors on the miniature battlefield.
Whenever a unit becomes stranded in this manner, the attacking unit is immediately nudged (by as small an amount as possible) to bring it back into contact with the foe. This move cannot be used to alter the facing the attacker is in base contact with, nor is it an opportunity to change the attacker's formation or charge a unit not engaged in the fight. If the attacker cannot be moved in this manner, then the defending unit is moved instead."
Question 3: unit A stays as it is, because it cannot execute a combat reform, and the enemy will retain the CR boni for flank and rear attack.
BRB p. 55: "If a unit is engaged to more than one facing (say to the front and one flank), it cannot make combat reforms."

That is correct, with one exception: all the models in the second rank of unit B behind a red model can make a supporting attack. Of course, in this particular case, unit A could make a combat reform to face unit B.
