Hatred grants a unit re-rolls to hit in the first round of combat. Presumably that applies to that unit's first round of combat, not the first round of "a combat".
To clarify, an example from my recent dwarf vs skaven game: the Ironbreakers and Clanrats were already engaged and had fought a round of combat. I then flank charged the Clanrats with my Longbeards. As far as the Longbeards are concerned, this is their first round in this combat - so I re-rolled misses - is that correct? Later when the Longbeards themselves got flanked by Stormvermin, I didn't get Hatred re-rolls because this wasn't the Longbeards' first round in the ongoing combat.

A: No to both questions.

Yes, that is correct. It is the first round for each unit in each combat.
