So for example:
Scyla 5x5 base - Monstrous beast Skull crushers 5x7.5 base - monstrous beast cavalry
As his base size is different should be positioned to the side of the unit instead of in front?
If I have 3 crushers and Scyla joins will he get look out sir?

He is placed to the side and no look out sir. They are not the sane category. One is cavalry. Just like cavalry and infantry are different.

How about dragon ogres? Same type but different base size. There are no Dragon Ogre characters which would get a Look Out Sir. Kholek Suneater is a named character and monster. A Dragon Ogre Shaggoth is also a monster so they go around by themselves and can not join a unit of Dragon Ogres. Shaggoths have thunderstomp, normal Dragon Ogres have stomp. A Shartak is treated as a normal unit champion upgrade so no lookout sir.

The only thing that matters for 'Look Out Sir!' is troop type. So a Monstrous Beast gets lookout sir if they join a unit of monstrous beasts, regardless of base size, models size, etc.
Scyla is unbreakable and so cannot join units.

BRB p. 97: "If a lone character is hit by a shooting attack of any kind (including shots fired using ballistic skill, templates and so on) a "Look Out Sir!" roll can be attempted, provided there is a friendly unit consisting of five or more models of the character's troop type within 3". Roll a D6. On a roll of 4+, the character has been successfully forewarned or otherwise preserved from harm by his nearby allies — the hit is transferred to a model in the friendly unit (if there is more than one eligible unit within 3", the controlling player can decide which made the honourable sacrifice). Otherwise, the hit is resolved against the character as normal."

Indeed. There is a difference between ranks and rank & File models. For Look Out Sir, it must always be 5 rank & file models (including the musician and standard bearer, but not the champion) of the same troop type. For Monstrous ranks you need only three monstrous models, whether they are R&F or not.

I would assume not since it specifically states 5 with no disclaimers (without looking at the book) I think the first 8th FAQ made it three for monstrous but that was a long time ago!
