The raised/summoned dead, do they consist of fallen warriors from the past?
If so, when they died, did they already wear the armour/symbols of the undead? Or did they swap it out before standing up again?

Heh, I struggled with that question when painting my Zombies' banner recently - typically they should have a winged skull or something to fit the Undead look, but would a zombie really stitch together a fresh banner to show their new allegiance? Or would they carry the same old banner proclaiming loyalty to some Empire province, as they did in life? The answer, of couse, should be that the magic of Undeath twists their armour and sigils into new forms better reflecting the corrupted nature of their Vampiric overlords.

Or did they swap it out before standing up again? The Vampire Count army books used to include rules for the Von Carsteins mobilizing the living population of Sylvania. It is conceivable that those (living) units are adorned with undead styled weapons, armor, banners, etc. If they die and are subsequently reanimated they would have the look we are all familiar with. But yea if you think about it in the vast majority of situations, freshly raised undead should not have batwinged helmets and bone heraldry on their shields. Does the necromancer spend months before a battle animating blacksmiths to make undead themed armor? Also where are the elf. dwarf, skaven, lizardman, and orc skeletons and zombies? Yea, I know about the Dogs of War Cursed Company. Still doesn't make any sense. Back on topic, those undead Warhammer Underworlds models are pretty amazing for plastic skeletons.

I'd say, GW never lets logic get in the way of a flashy model. According to fluff, Dwarfs, Elves, Orcs and even a Chaos Champion, have been raised from the dead, although I should think that most Undead were humans. For verisimilitude (if that word may be used in this context), you could always convert other armies into Undead. In the 6th edition Vampire Counts AB, Daniel Tunbridge did so with Empire models.

It could be that...
Dwarfs are resistant to magic, even in death.
Orcs/goblins are mushrooms, and so dissolve/rot rather quickly.
As Elves are T3 throughout, when they die, they explode into blood and dust.
Lizardmen, Beastmen and Skaven are too complicated anatomically, for the undead to ressurect. (else there could be a Skaven or Slann Vampire for example?)
